Why it matters
Scaffolds put whole crews at height on a structure someone assembled that morning. Most scaffold accidents trace back to missing planks, missing guardrails, or an inspection that never happened. The competent person’s green tag is what stands between routine and disaster.
Hazards
- ⚠ Missing guardrails, midrails, or toe boards at open sides
- ⚠ Gaps in planking or planks not extending properly over supports
- ⚠ Scaffold not inspected after weather or modification
- ⚠ Base plates on soft ground or missing mudsills
- ⚠ Climbing the frame instead of a proper ladder or stair
- ⚠ Tools and material falling on workers below
Controls and safe practices
- ✓ Competent person inspects before every shift and after any change or storm.
- ✓ Full planking, no gaps over 1 inch. Planks extend 6 to 12 inches past supports.
- ✓ Guardrails at 10 feet: top rail, midrail, toe board on open sides.
- ✓ Firm foundation: base plates and mudsills, never blocks or debris.
- ✓ Access by ladder or stair tower only. Never climb cross braces.
- ✓ Barricade below and use debris nets or catch platforms where others work underneath.
- ✓ Keep platforms clear of ice, mud, and stacked material that blocks the walkway.

- Are all sections fully planked and railed right now?
- What is coming up that will require modifying the scaffold, and who does that?
- Who is working below the scaffold today?
