Does OSHA require a written PPE hazard assessment?

Yes. Under 29 CFR 1910.132(d), employers must assess the workplace for hazards that require PPE and certify that assessment in writing, identifying the workplace, the person certifying, and the date. This tool produces that written certification.

What must the certification include?

The written certification must identify the workplace evaluated, the name of the person certifying that the evaluation was done, and the date. Good practice also lists the hazards found and the PPE required for each, which this tool includes.

Do I still need training and a program?

Yes. The assessment is one requirement. Employers must also select and provide the right PPE, train affected employees on it before exposure, and, for respirators or hearing protection, run a full program. A written safety program ties these together.
