Alberta orders
Alberta: The employer must prepare a report of the results of the hazard assessment and the methods used to control or eliminate the hazards identified. (OHS Code, AR 191/2021, s. 7(1)-(4))
Pick Alberta and the document adds that report, filled in with your details. For other provinces and territories the document quotes no FLHA rule and says so.

When do I enter my details?
